Should the drum be replaced when you replace the cartridge?

No. You do not need to replace the drum with every toner. Drum last about 3-4 toner cartridges. Replace when machine will ask you to replace or when the print problem does not go away when you replace the toner

I have a Brother HL-2270DW printer. Can I use Brother TN-450 for it, What is the different between TN-420 and TN-450? I know 450 is supposed to be High Yield Toner, do they both fit my printer?

TN-450 and TN-420 are the same cartridges. Tn450 is high yield and has more toner. Any machine that uses TN-420 will take TN-450 as well.
